New! MediSystem Pharmacy – Chronic Obstructive Pulmonary Disease Protocol Optimizing Therapy for Residents in Long Term Care

MediSystem Pharmacy is pleased to support 'Breathe Well in Long Term Care', a new program designed to optimize treatment and management of LTC residents living with COPD. COPD affects 1 in 7 residents in LTC, with prevalence rate of 16% in 2014-2015 (CIHI). COPD has a tendency to progress over time, and is associated with frequent exacerbations, hospitalizations and premature death (GOLD 2016, CTS 2007).

This pharmacy driven COPD protocol was developed in partnership with a respirologist from St. Joseph’s Healthcare, to be utilized by care providers to effectively assess and manage LTC residents living with COPD. The protocol serves as a guide to identify areas where target goals for treatment can be improved. The desired outcome is to improve quality of care for residents, minimize hospital transfers through proper use of pharmacotherapy, and reduce exacerbations and related complications through appropriate symptom management.

The long term objective of this initiative is to offer a program that is both scalable and sustainable, and to implement an interdisciplinary approach to shift COPD management from the perspective of acute rescue to one of proactive maintenance.
